POSITION TITLE: Intake Specialist
Reports to: Director of Clinical Services
POSITION PURPOSE: The function of the Intake Specialist is to perform initial assessment of each patient to identify individual’s needs and determine HOH’s ability to meet them.
ESSENTIAL JOB RESPONSIBILITIES:
- Conducts initial patient interviews, gathering background, financial and other demographic data to be utilized in making eligibility determinations, referrals to specific programs within HOH and/or referrals to other agencies, programs or resources.
- Interacts with referral sources to facilitate communications, answer questions and resolve problems.
- Researches/identifies all potential payer sources and determines the primary payer.
- Documents demographics/clinical payer information and determines coverage availability for requested services.
- Records the outcome of all inquiries and referrals calls received.
- Contact the attending physician regarding any patient that the Intake Specialist feels should be admitted to any level of treatment.
- Coordinates internal activities to ensure a smooth transition for the patient into HOH’s care.
- Perform basic clerical duties (i.e. filing, telephone calls, copying, faxing).
- Operates computer to enter and retrieve data and type correspondence and reports.
- Maintain customer information in confidential manner.
- Attends hospital orientation, staff meetings and completes all mandatory yearly in-services.
- Perform all other duties as assigned.
MINIMUM QUALIFICATIONS:
- Master’s Fully Licensed Social Worker.
- Effective interpersonal communication skills.
- Basic clerical, communication and organizational skills.
- Ability to work independently and without constant direction.
- Ability to work within a team environment.
- Be able to maintain a professional appearance and attitude when dealing with clients and coworkers
